D02C Disclosure Statements - Corporate

When you utilize the six (6) page Disclosure Statements (D02/D02C) document in your client meetings, it reduces the need to obtain signatures on more than one form and simplifies your sales process:

 

  • Insurance related sales, a Needs Analysis (A04) incorporates a signature box with date of delivery; OR
  • Investment sales, Know Your Client (KYC) (A03) incorporates a signature box with date of delivery; OR
  • Acknowledgement of Receipt (A08) - If you utilize a different Needs Analysis/KYC, utilize the Acknowledgement of Receipt (A08) form to collect client signatures. We require the actual mandatory client documents along with the A08 form as proof of signature. The A08 form is a tool to collect your client signatures, it does not replace the need to upload the mandatory forms.

D02/D02C disclosure incorporates the following topics to meet industry requirements:

  • Privacy (A01/A01C);
  • Consent to electronic delivery of communications (CASL) (A10);
  • Consent and Authorization (A02); and
  • Client Complaint form (D05)

IMPORTANT:  When a one (1) page Disclosure Statements is utilized, all the other additional documents (noted above) are required to be handed out to cover mandatory disclosure and obtain client's signatures to confirm same.  An Acknowledgement of Receipt (A08) can be utilized in combination with a different disclosure - provided you obtain the A08 acknowledgement..

 

TIP:  your client file must include a signature (or initials) to verify you provide all your Disclosure Statements document to your client.  This can be easily be accomplished by having the client initial the boxes on page 2 of the Needs Analysis form, or alternatively you may collect client signatures on the A08 form, Acknowledgement of Receipt (A08) for your convenience.
